Previously, Flolan was used as a "bridge" to transplantation but now with the evolution of successful medical therapies, transplant is limited to those patients who fail maximum medical therapy. Finding the best "window" for listing a patient for transplantation is often a difficult process. The benefits and challenges to the new allocation system will be reviewed, with emphasis on the impact on PAH patients. Survival after transplant remains below optimum, but can provide years of improvement in quality of life.
