This course features 3 sessions from the 2011 PH Professional Network Symposium that focus on issues of special interest to pharmacists. These topics include transition of medications between inpatient and outpatient care, transitioning patients between medications and medication safety. This course is offered for a maximum of 3.0 hours of pharmacy CEUs.
